Housing Stabilization Services
Eligibility: Individuals with a mental health condition or disability who are homeless, at risk of homelessness, or struggling with housing stability.
Eligibility is also determined based on income levels and other DHS criteria. Benefits: Assistance securing and maintaining safe and affordable housing, guidance navigating landlord-tenant relationships, and prevention of homelessness.
Home and Community-Based Services (HCBS)
Eligibility: Individuals with disabilities or chronic health conditions who require support to live independently in their community. Determination of eligibility is based on the DHS’s assessments of medical necessity and level of care required.
Benefits: Enhanced independence through assistance with daily activities, improved access to community resources and engagement, increased self-reliance, and overall better quality of life.
